Hysteroscopy

Hysteroscopy is a procedure in which a thin camera (hysteroscope) is passed through the vagina and cervix to look directly inside the uterus. It is used to diagnose and treat uterine polyps, fibroids inside the cavity, scar tissue (Asherman syndrome), a uterine septum and abnormal bleeding. At Janisthaa’s Basaveshwaranagar hospital, Dr Shwetha Y Baratikkae performs diagnostic and operative hysteroscopy as a day-care procedure, with no cuts on the abdomen.

What Is Hysteroscopy?

A hysteroscope is a thin, lighted telescope, usually 3–5 mm wide. It is gently passed through the cervix while sterile saline fluid expands the uterus. Your doctor can then see the uterine lining on a screen and, if needed, remove abnormal tissue in the same sitting using tiny instruments passed through the scope. Because it enters through the natural opening, there are no abdominal cuts and no stitches.

Types of Hysteroscopy – Diagnostic vs Operative

 Diagnostic hysteroscopyOperative hysteroscopy
PurposeTo look inside the uterus and find the cause of a problemTo treat what is found
What’s doneInspection, with or without a small biopsyRemoval of polyps, fibroids, scar tissue or a septum; D&C
AnaesthesiaNone, local or short sedationShort general anaesthesia or sedation
Duration10–15 minutes20–45 minutes
Hospital stayDay-care, 2–4 hoursDay-care, 4–6 hours
Back to normalSame or next day1–2 days

Office hysteroscopy is a quick diagnostic hysteroscopy done in the outpatient clinic with a very thin scope, usually without anaesthesia. Where findings need treatment, a diagnostic hysteroscopy can often be converted into an operative one in the same sitting, so you don’t need a second procedure.

When Is Hysteroscopy Recommended?

Dr may advise hysteroscopy if you have:

  • Heavy, irregular or bleeding between periods
  • Bleeding after menopause
  • A polyp or fibroid seen on ultrasound
  • Repeated miscarriages
  • Infertility, or repeated IVF or IUI failure
  • Very light or absent periods after a D&C or abortion (possible Asherman syndrome)
  • A suspected uterine septum or other shape abnormality
  • Retained pregnancy tissue after a miscarriage or delivery
  • A lost or embedded IUD (Copper-T)

Uterine Polyp Removal (Hysteroscopic Polypectomy)

Endometrial polyps are soft, usually non-cancerous growths of the uterine lining. They can cause irregular bleeding and may interfere with embryo implantation. In hysteroscopic polypectomy, the polyp is removed under direct vision, right at its base, which lowers the chance of it coming back. The tissue is sent for biopsy. Most women go home within a few hours.

Asherman Syndrome Treatment (Adhesiolysis)
Asherman syndrome is scar tissue (adhesions) inside the uterus. It usually follows a D&C, a surgical abortion or a uterine infection. Typical symptoms are very scanty or absent periods, repeated miscarriage and infertility. Hysteroscopy is the only way to both confirm and treat it. The scar bands are carefully cut under camera view to restore the normal cavity. To stop the scar tissue re-forming, Dr Shwetha may use estrogen therapy, an intrauterine balloon or a gel, and plan a short second-look hysteroscopy to check healing.

Hysteroscopy and D&C
A D&C (dilatation and curettage) gently scrapes the uterine lining to remove tissue or take a sample. Done “blind”, it can miss small lesions.

Hysteroscopy-guided D&C
lets the surgeon see exactly what is being removed, which makes it more precise and gentler on the healthy lining. This matters for future fertility, because rough curettage is a common cause of Asherman syndrome.

Septum Resection and Submucous Fibroid Removal
A uterine septum (a wall of tissue dividing the uterus) and fibroids bulging into the cavity can both cause miscarriage or implantation failure. Both can usually be removed through the hysteroscope, without any abdominal surgery.

Hysteroscopy vs HSG vs Laparoscopy – Which Test Do You Need?

Laparoscopy and hysteroscopy together: for women with infertility, both are sometimes done in one sitting, checking the inside and outside of the uterus and the tubes under a single anaesthesia.

Hysteroscopy Procedure – Step by Step

Before the procedure

  • Best timing: usually just after your period ends (around day 6–11 of the cycle), when the lining is thin and the view is clearest.
  • Tests: pelvic ultrasound, a pregnancy test, blood tests and, if sedation is planned, a fitness check.
  • Fasting: 6 hours without food if anaesthesia is planned.
  • A medicine to soften the cervix may be given a few hours before.

During the procedure

  1. You lie in the same position as for a pelvic examination.
  2. The hysteroscope is gently passed through the cervix. No cut is made.
  3. Saline expands the uterus so the lining can be seen clearly.
  4. Dr Shwetha examines the cavity and the openings of the tubes.
  5. If a polyp, adhesion or septum is found, it is treated with fine instruments.
  6. Any tissue removed is sent for biopsy.

After the procedure

You rest for 1–3 hours and go home the same day. The doctor explains the findings before you leave, and biopsy results are reviewed at your follow-up visit.

Hysteroscopy and Fertility – Does It Improve Your Chances of Pregnancy or IVF?

When hysteroscopy finds and treats a real problem, such as a polyp, septum, submucous fibroid or adhesions, it can remove a barrier to implantation and improve your chances of conceiving. However, a large randomised trial (inSIGHT) found that routine hysteroscopy for every woman with a normal ultrasound, before a first IVF cycle, did not increase IVF success. That’s why Dr Shwetha recommends hysteroscopy only when there is a clear reason: abnormal scan findings, repeated IVF or IUI failure, recurrent miscarriage or suspected Asherman syndrome.

When can you try for pregnancy or start IVF afterwards?

  • After diagnostic hysteroscopy: usually from the next cycle.
  • After polyp removal: usually after 1 cycle.
  • After septum resection or adhesiolysis: usually after 1–3 cycles, once healing is confirmed.

Is Hysteroscopy Safe? Risks and Side Effects

Hysteroscopy is one of the safest gynaecological procedures, and serious complications are uncommon. Possible risks include infection, bleeding, a small injury to the uterine wall (perforation), fluid-related complications during long operative procedures, and reactions to anaesthesia. Careful technique, fluid monitoring and an experienced endoscopic surgeon keep these risks low.
