TESA is a treatment method employed when infertility results because of the male partner. It is one of the sperm retrieval processes that is carried out in cases of Obstructive Azoospermia or lack of sperm in semen. It is an invasive technique that involves extracting the sperm form the tubules of the Testis.
As mentioned, TESA is used in cases of poor sperm production which could be due to genetic issues, infections, maldescended testes etc. Obstruction of genital tract that is irreversible due to previous surgeries or trauma or infection could also be treated with TESA. A previous vasectomy or absence of vas deferens by birth are also some of the common reasons when TESA is used.It is also used in combination with ICSI or IVF. The success rate for the procedure is similar to IVF and ICSI.
It is performed in a hospital set-up, under general or local anesthesia. Pre-testing might include blood tests and testicular biopsy. Unlike in PESA where a fine needle is passed through the skin of the epididmysis to retrieve the sperm, here, the needle is passed through the skin directly into the testes to collect a few small number of seminiferous tubules themselves. Once collected, the tubules are dissected in the laboratory to search if any content of the sperms are present. This is because sperms are produced in the testes in these tubes called the seminiferous tubules. TESA does not require hospitalization. The person can be discharged on the same day.
